So I'll give them the benefit of the doubt and say the designers probably knew they were doing something weird, right? Do you think they just decided not to care? I mean, they couldn't possibly do that without seeing it at all... Could they?

Some people cry out to God for an explanation when bad things happen; I apparently cry out to Stefan. Does that make me a Stefanist?

My *guess* would be is that they came up with the concept for the poster (Scarlett plus kid), which given what the movie is about makes total sense And that then they were informed that Laura Linney's name was contractually obligated to appear above the title. Often what's in stars' contracts dictates what's on the poster, which is also why you get a lot of movie posters where the names of the people in the film do not line up over their heads (because it has to be alphabetical or in a certain order or whatever).
Loathe to change the poster concept, I bet you they just figured nobody would really care and made the change fit as best they could.
